How do I run the Forms Completion Report?

  1. Hover over the Roster tab
  2. Click on Roster List
  3. Use the checkboxes to select the student-athletes you wish to appear on the Forms Completion report OR Click the select all checkbox
  4. Use the Reports dropdown on the right of the screen to select *Form:*Completion Report
  5. Click on the Printer  icon
  6. This report will generate the list of selected student-athletes in Microsoft Excel. There will be a column for each form in your ACS System. The date shown is the last date the Student-Athlete electronically signed the document.
  7. Sort & Search through Microsoft Excel document as necessary
